Midlands Partnership University NHS Foundation Trust provide a memory and dementia care pathway across South Staffordshire that provides specialist help for people with memory problems. The service offers specialist memory assessment, diagnosis and treatment. During your initial assessments they will meet with you, your family / carers to discuss your current difficulties and agree on a plan of care to address your ongoing needs. Referral should be made through your GP.
